    How can I convert my Adobe FormsCentral form and keep the same layout?

    It's normal some style changes. From what I see the fields are there, it's just the position that is different. You may try to shrink the fields so that they stay in the same line:

    Then you can drag and drop the fields so they can align in the same line.

    It is possible that the fields need some width adjustment. You can do that by selecting form designer > css > double click the field and then adding some css code:
