my account was suspended.
than my account was reactivated.
but now are my forms disabled - please enable my forms.

Upon checking your account, I did not find any issue with that. It seems one of my colleague have already taken care of your account issues. Your form is also active and you should not have any trouble. I was able to send a test submission successfully. Please check the screenshot below:
