How to create a form that has narrative fields?

    Is there a way to have a form say a message and then add the fields entered after the message?

     

    example.

     

    Hello I would like to submit a new location for our training sessions.  The location is (field data goes here)

    There's a widget that can definitely help you achieve your desired form.

    Please try and make use of our Narrative Fields widget.

    The Email Notification that you will receive will look like the image below when someone fills up your form and submit it.
