Where is my form?

Hello I created this form last year: http://form.jotformeu.com/form/40548923670358

and I need to update it but it is not visible when I log into jotform. 

The submissions are still there in google Drive and there is a copy of the form also in Drive, but when I right-click on it  and selct 'open with jotform' it takes me to jotform and there is no sign of this particular form.

That form is associated with the username "r.irving.worc" and the email address "r.irving.worc@gmail.com".  It seems the form you referenced is the only active form in that account. 

If you would like, we can move that form into your current account.  Just log in to the "r.irving.worc" account and let us know you would like that form moved to your "rirving" account.
