Form doesn't look the same way in the builder as when it's viewed on a browser

     I am creating a form basically for people to rate you-tube videos via a star rating and answer a few questions.  When I view the actual form, the videos are not in the same order, they are duplicated, or the videos are missing compared to what I am seeing on jotform.  I make sure it is saved before I test.  I have uploaded a screenshot.

    Can you please check this form: http://form.jotformpro.com/form/51105004000932 and let us know if it works ok on your side? It's a cloned version of your form, but I made some changes in the fields to shrink them and positioning them in separate lines as detailed here: http://www.jotform.com/help/90-Form-Field-Positioning

    If this form works for you, feel free to clone it to your account to inspect it further.
