the PDF version of the form looks bad

Please look at the original form I transferred from Adobe...when I download it as a pdf in your system this is what I get. Not attractive at all...

My question is...would all my issues disappear if I just embedded the form instead of using PDF's?

Though my copy of your PDF form did not seem to be nearly as bad for some reason:

The PDF copy will always be a bit degraded in appearance.  This is due to much of the styling of the original form not being translated to the PDF copy. 

However, the embedded version of your form will look the exact same as the original.  If this is an option, it will be a much cleaner copy of your form than the PDF.
