It appears as though you have upgraded a Guest account. The first thing to do would be to register your account. Try registering using the same email address you used to create your forms "email@example.com". If it will allow you, then your forms and subscription will be transferred automatically. If it will not, register using another email address, then let us know you would like the forms and subscription from "guest_51067612819053" moved to your registered account.
Once you have your account registered, you will then be able to create PDF versions of your forms from you account. Just select the form you wish to create a PDF version of, then click on "Create PDF Form" under the "More" menu:
