Can I create something that looks like this with the one of the tools and with a click of the button - looking to do it in one part as opposed to a part for the yes/no and one part for the comments

Unfortunately, we cannot combine multiple field types into a single input. It is possible to add different field types as groups with a Configurable List widget, but the fields will be still separate from each other.
We would like to suggest using standard fields. These fields can also have conditional logic attached to them.
