Upon checking your account, I did not find any issue with that. Your forms are not deleted and there are 4 forms in your account. Please check the screenshot below:
Are you not able to see forms in your account? It could be browser related issue. I would suggest you to clear your browser cache and see if that solves your problem. The following guide should help you in clearing browser cache: http://www.wikihow.com/Clear-Your-Browser's-Cache
