Form is giving "field is required" message on zip code, although it's filled in

I have address set to required.  When I fill in the zip code, it gives an error that "this field is required" even though the zip code has been entered.  It will not let the form be submitted.

That error message is associated with all the address field, not specifically with the Zip code.

As the field has 2 address lines both need to be filled in. If you want you can remove some options from the address field:
