Is it possible to add a button to cancel and close the form next to the submit button?

@Jason, yes, it is absosutely possible to add a cancel button.
The process is illustrated on the following figure:
1 - From your form, click on the Submit button area.
2 - The panel of the top will become activate, click on Reset Button and a Clear Form buttom will show:
I hope this helps.

That only CLEARS the form. I'd like the button to CLOSE the form. Is that possible?

You should be able to do something like that with a custom js code to your form for that button.
Something like this should work: "window.close();", but it would be best to test out and hire a developer to do what you want to happen when Cancel is clicked - since we can not assist you with custom JS code.
Hope that helps.
