Can't seem to explain how but now it appears to be viewing OK.
All I did was remove the "Captcha" widget - save - then reloaded it. Still rather strange. Thnx for viewing.
My form appears OK on Windows based browsers and Safari [ iPad ].
The issue is with the iPhone. The main login page for Jotform appears in place of the form I intend to display. Wierd.

Which browser and version are you using on iPhone?
I tested but could not replicate the same situation. But you mention that when you removed the Captcha widget it started to work normally.
It might have been a temporary bug with the Captcha widget.
Let us know if the problem reappears.
