Changing form on Facebook page

    choosing forms from app box on facebook page

    Can you tell me why the wheel icon to choose a particular form in the app box on the facebook page keeps disappearing and what i need to do to correct it.

    Can you please be more specific in your request?

    I saw that you have form embed to your Facebook page but not sure where you do not see wheel icon. More settings between your form and Facebook page I can not see because I am not logged like admin of that page or someone who can change settings related to JotForm form. Inside of this article at the end you can find part where it is described how you can change the form on Facebook page.
