    How do I add your banner to my site? And questions about radio buttons.

    In your FAQs, you mention adding your banner to my site but I didn't see any information on how to do so.

    Also, can someone using the contact form select more than one radio button or only one?

    Finally, I wanted to know if there was a way to add predetermined text plus text supplied by the person wanting to contact me to form the subject line of e-mail sent to me to make it easier to identify in my e-mail inbox.  For example, my contact form would include radio buttons.  In the "Question" field, I wanted to put "Reason for Contact" followed by "Reason 1", "Reason 2", and so forth in the "Options" field and then selecting "Allow Other".   Then, if the person selected "Reason 1", the subject line of the e-mail would read "Predetermined Text" - "Option 1".

    Hello Jon,

    If I understand your question correctly, you want to add an image in your form. Is that correct?

    You need to just add image field in your form. It will display an image wizard and you need to follow the steps to upload the image. Please check the screenshot below:

    No, I wanted to add your banner to my website to spread the word about your service.  As per your FAQ:

    "To support our free service, please feel free to add this banner to your site"

     

    Do I link to that image or should I download the image and then upload it to an image hosting site?  If the former, you might want to consider supplying HTML code on the FAQ page so that people can copy and paste it onto their websites/blogs/etc.

    Sorry about the confusion.

    You do necessarily have to download the image and link it in your web page. You can use the following html code and place it in the desired location of your web page:

    <a target="_blank" href="http://www.jotform.com"><img src="//www.jotform.com/images/jotform.gif"></a>
