My Form's have been disabled :(

Hi there, i have recently created three different forms for my HTML website for a school project. They were working fine while i had them, however now when i go to test them the loading states "Form not found; this form has been disabled". Even after looking at the form settings and ensuring that the forms were infact enabled they still fail to work!! Please help my website is due soon and I need these forms to work!!

It is disabled because you are asking password in "Console Wars - Registration" form. Our system thought your form is phishing. That is why it was suspended. It is active now. You can use it. Please remove the password field.
