    Hi,

    For Input Boxes, please use this:

    .form-all input {

    background: #969C62;

    }

    The Code that you are currently using will only color the Input Text not the boxes.

    For the Header/Sub-header border and background, please use this:

    .form-header-group {

    background: whiteSmoke;

    border-bottom: 1px solid #CCC;

    }

    .form-subHeader {

    border-top: 1px solid #CCC;

    }

    And for the Textbox Color, use this:

    .form-all textarea{

    background-color: white;

    }

    This will also remove the form active background:

    .form-line-active{

    background:transparent;

    }

    I have compiled all the codes so you can copy it here: http://pastebin.com/cq3H7Bxc

    I think I have already added all the CSS Codes that you have pointed an arrow on the provided screen. Just replace the colored text to any color you like.

    I hope this helps. If you have further questions, just let us know. Thank you!
