Form Not Found. This form is disabled message

I had added the registration form link to to the Register Here prompt on my website.  It worked the first time i tested it and now I'm getting the disabled message.  Then I tried the online Waiver form and I'm getting the same thing.  What's going on?  How do i fix it?
