I'm researching your form and I don't immediately see any problems. I checked your email address and it's not on our bounce or unsubscribe lists. There also aren't any error messages in our log files.
We allow users to check their form email history. You can check them here. I didn't see any errors in your account there either.
Here's the most recent one that looks like it left our server correctly.
I see you have four notifications set up. Are the other three being delivered successfully? It's possible that this email provider is marking the message as spam. Is firstname.lastname@example.org being forwarded to a gmail account, perhaps the same gmail account as one of the other, notification email addresses? I don't see anything wrong on our end. With email though it's sometimes blocked once it leaves our servers.
