How can I have the text to show in the submitted form?

I am trying to create a Photography Model Release form and I would like for the legal text to show on the submitted form. Not just the input fields. How can I do that? Now when I use the "text" blocks, they show when the form is being created but disappear when the form is submitted. I would like to have the text shown in the final submitted form.

The short answer is you need to customize your notification to include the text you would like. We don't include static information in submissions, as usually it's not useful. It's easy to customize notifications though.
