Why isn't embedding a JotForm working on a WordPress site?

I have created a website using WP 4.2.1.  Also, I have created a JotForm and attempted to embed the form and use the WP plugin for JotForm to embed the form. Unfortunately, it isn't working. In fact, it's making that particular page perform in a weird fashion.

Whenever I visit the page with the form embedded, it duplicates the page and the form never appears.

    Hi,

That's probably due to script conflicts. There are lots of jQuery based plugin in your WP which would possibly conflict the form. I would suggest to re-embed your form using iframe embed code. This should resolve the problem.

Hope this helps. Contact us again whenever you need further assistance.

    Thanks!

This make sense...I would like to do two things,

completely align the form to the left so that it falls directly under the text on the webpage
    .  Secondly,
I would like to update the font to match the font on the website.
Please advise on how to do these two things.
