Creating a multi-line entry form (spreadsheet-style)

I need to have a multi-field entry form, and can't figure out how to do that.

This is what I need in a nutshell:

5 columns, each line to be populated as needed (minimum of 1 line worth of entries, maximum of 5 lines)

2 columns: text entries, 1 column - numeric year entry, 2 columns - Yes/No entries

    i.e.

Text Field 1 (Description)        Text Field 2 (Description)         Year Field (From-To)         Yes/No Field    Yes/No Field

    Line 1

    Line 2

    ...

    Line 8

I have no idea how to even approach doing this.

    Thanks in advance!

    Hi,

You can do all of that using this widget, Configurable List.

What is so much special with this is that, users can add as many rows as they pleased since they can be created dynamically within your form.

    Example: http://form.jotformpro.com/form/43222928180957?

Let us know should you need further assistance. Thanks!

    Thank you!

As always, very quick replies by JotForm Support Team. Very much appreciated!

    However,

I am having trouble editing my Configurable Lists past the point of creation - at some point all but the first entry/field disappear without a trace.

    Is this a known issue?

How to undo something like that?

    Thank you in advance!

You are welcome. Glad to see that Configurable list widget works for your requirement. I've moved your question with the issue editing the configurable list to a new thread and shall be addressed there shortly.
