Is it possible to work with 1 form on multiple sites but to know from which site my form filled form is ?

Hello, 

I use 1 form on multiple sites. When submission comes I'd like to know from which site the form was filled.

Is this possible and how or do I have to clone more forms ?

As long as your are not using the iFrame method to embed your form, adding the Get Referrer widget to your form will let you know the page your form was submitted from:

http://widgets.jotform.com/widget/get_referrer
