Is it possible to use the form source to post discussions?

I am trying to use the html form to post discussions.

However, there are only three fields that are showing (the date and name fields) but the answers are not posting.

Also, please let us know which form on your account you are trying to use this workflow with since you have a lot of forms and I am unsure of which you are intending to use for that purpose if you have one set up already.
