Need help in creating online forms for my website to replace PDF links

    I run the Pro Ball Baseball Clinic (pro-ball.com). I presently have 4 "writeable" waiver forms on my website that clients need to fill out and then scan/fax/mail back to me.

    I want to change those forms so that clients can fill them out on-line AND send directly to my email. Bottom line, I want to make things easier for them.

    I also have a questionnaire, not on website, that I want uploaded to the website that can be completed on-line and directly sent to my email.

    Can you help?

    Thank you for your interest in using JotForm. I've checked your PDF forms on your webpage and these forms can be created using JotForm. Creating forms using JotForm is easy and quick by using drag and drop method. You can create fantastic forms using our Form Designer and use a theme from our theme-store.

    Once the form is created using JotForm, you can easily embed on your website. Also, there are several other features like accepting payments through JotForm, perform calculations, send email notification when the form is filled, and many more at one place.
