"Take Photo" does not work. It displays black image.

I have created a form that allows users to upload photo.

However, it does not load.

Pls see black image.  What can be done to ensure that it loads properly every time?

I did check your form and did not find any issue with that. The "Take Photo" widget you have added in your form seems to work as expected. 

Please be noted that when the form with the Take Photo widget is loaded in the form, it will not be able to access the camera of your laptop or other devices. It will ask you for permission and you will have to allow it. Please check the screenshot of your form below and it seems to work just fine:

If you allow your browser / form widget to access the camera device, the widget will work just fine.
