The model search form I created is unavailable and is not showing up on my website. Why is this and will it come back?

I did check your web page where you have embedded your form and it is very much visible online. It seems to work just fine and I was able to submit the form successfully. Please check the screenshot below:
I would suggest you to test your form again and get back to us if you have any trouble.
There are few other things I wanted to point out :
#1. You seems to only have a guest account. This type of accounts are created if you do not sign up / sign in to your account and start creating forms. I would suggest you to "Sign Up" for a free account using your email address email@example.com and all your forms should automatically move to your new account. If you do not see your forms, do get back to us with your new username and we will move the forms manually.
Please be noted that in guest account, you can only receive 20 submissions in a month but free account will give you 100 submissions / month.
#2. I see that you have several form fields in your form which should be hidden. It should be displayed only based on the answer of previous question.
IF "Are You Currently Working With a Modeling Agency?" IS "YES" THEN ONLY DISPLAY THE FIELDS "If Yes, Name of Agency:" AND "Agency Contact Person"
