Unable to receive email notifications

MCFSL Scores Reporter. I am not getting the emails and the FAQ references and "Email Alerts" button, that is completely missing from what I see.

Upon checking the form, I see the "Email Alerts" button and your notification there.

I also see that there was a submission received by the email scores****@chu***s**tb**l.org, there was no failed email attempts in our mail logs and that email is not in our bounce list. Did you not receive the submission in that email? In that case, your email service provider might be blocking incoming emails from our end. Please request to them to whitelist our IP addresses and domain names, here is the list that needs to be whitelisted: http://www.jotform.com/help/145-Whitelisting-JotMails-IP-Addresses.

Also, when making test submissions, you'll need to open the form in live view, fill it out and click the submit button.
