I need to add some fields and I need help

I have provided an example of what I need to do. I had it done before, but I have forgotten how I did it and I have tried for hours to figure it out. Can someone please help guide me in the right direction its literally the last thing that I need to get done. I can figure out how to get the dropdown boxes and add text and options in those but actually getting the text outside of the box, such as the 'check box' option at the top and gray 'Camper type' to the right side is confusing me.

You have to configure that using our Payment Integration:

The checkboxes are your product items. The gray Camper Type text is your Custom Property Name.
