How to attache formula for each selection

On my intake form I have a list of symptoms for my patients to check. I would like to attach a formula to each symptom they check that will only show to me. What is the best way to do that?

As is now, I can only base my solution on the assumption that this is what you meant.

You can use Smart Forms Using Condition Logic to Show/Hide and Update field content on the form.
