How can I have text fields displaying on PDF submission file?

Hi,

I have created a new form and have customers filling it out ok. When I go to download a pdf copy of it, all it shows is the fields that were filled out and not the rest of the verbage on the form. How do I get my saved pdf copies to show the whole form with all the writing that was on it? Thank you. Bob Piazza
