How to show headers inside of PDF submission

How can I get my headers to print out in my forms when I save them to a PDF? 

Also, the map doesn't render in a PDF export in my address fields. It looks fine on-line but the print out doesn't have all the information.

If you want to to Show Headers and Texts inside of your PDF file, you need to enable that option inside of Submissions. 

Regarding a second part of your question, can you please explain in more details what map doesn't render in a PDF export in your address field?
