This is the link I give my clients to fill out a questionnaire and it isn't working. http://valoryjeanphotography.com/wedding-questionnaire-3/

I've never had this happen before. Did something change? I need it fixed ASAP. Thanks!

After inspecting your website http://valoryjeanphotography.com/ seems that you do not have problem with JotForm. There is a problem with location of all of your pages. If I click on all of pages on your site I will get 404 error.

The 404 or Not Found error message is an HTTP standard response code indicating that the client was able to communicate with a given server, but the server could not find what was requested.

The web site hosting server will typically generate a "404 Not Found" web page when a user attempts to follow a broken or dead link; hence the 404 error is one of the most recognizable errors users can find on the web.

Seems that you have broken/dead links on your site. Maybe you moved your pages on some other location and you did not change URL of link inside of the site?

Considering your form, form is available http://www.jotform.us/form/30155738901150 and it is working.

Hope this will help. Let us know if you need further assistance.
