    Why have you disabled all my forms? Please reactivate my forms.

    Your account is disabled because on this form: http://www.jotformeu.com/form/30755475339361 you're requesting from credit card information without the use of a payment tool and this is against our terms of use.

    You must delete the culprit fields from this form and refrain yourself from adding them again or create another form asking for sensitive information in order to get your account reactivated.
