    Can you let me know why my form at http://form.jotformpro.com/jsform/40496827212961 is currently unavailable?

    Hello Susan,

    I did check your form and did not find any issue with that. It seems to be working fine. It seems you have already taken care of this issue. You have set your form's "Submission Limit" as 500 now. Please check the screenshot below:

    It seems you earlier had less submission limit and once you reached that limit, your form was automatically disabled. 

    I would suggest you to test your form again and get back to us if you have any issues.
