Cannot preview - Form not found

I am just building a form and have been unsuccessful when trying to preview it. I only receive a "Form not found" message. Since I am a rookie I suspect I am doing something wrong or the form has something that is not allowed??.  

I checked your account  and you had password fields which is not allowed according to our terms:

I deleted the password fields and activated your account. Creating forms with password fields will result in the suspension of the account.
