Forms no longer working

I created an employment application for our desktop website and the mobile website. Everything was working ok through 4/25/15 - the last entry on our google drive spreadsheet. I made some changes around that time but then copied the new embed code and placed it on the website. 

We have several positions open and many applicants have called us with submission problems.

I am not sure if I have understood your question correctly.

Do you mean to say that your users are not able to submit your embedded form or you are not receiving submission emails.

I did test your embedded form and it seems to work as expected. I was able to submit your form successfully. Please check the screenshot below:

I also check your form's notification email alert and did not find any issue with that. I check our email logs and found that they are being sent successfully. Please check the logs below:

The submission emails for my test was also sent successfully. Please check the screenshot below:

Can you please speak to your users and find out what issues they are having? Are they getting any error message? You may also ask them to send a screenshot of the issue. The following guide should help you how to upload image in forum post:  http://www.jotform.com/answers/277033
