Well, all your form fields are not required. When a field is not required, someone can just skip it and submit the form. You may consider making the field as required to see if that fixes the issue. For example, click the field and click "Required" button:
I would also suggest embedding the form using our iFrame method (https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code) if you have the form embedded to your website.
