I have created a form and am getting responses. However, on the PDF and excel versions of the responses, the "questions" or "labels" do not show up with the answers, so it is difficult to see what field goes with what answer unless you have the form pulled up in front of you.
How do I label the fields to make it clear on the PDF and excel reports which questions go with each answer?
The form is titled No Sweat Load Management Switch.

If you would like to show the text, headers and other non input fields in your PDF submission, you can do that by following this guide: http://www.jotform.com/help/305-How-to-show-headers-or-text-for-contracts-terms-agreements-in-the-PDF-format-of-a-submission.
Unfortunately, the text, headers and other non input fields or those without a value on it will not be displayed in the excel report, this is because the excel only fetches those input fields, converting their labels to columns and their values into rows.
