I need three boxes after a text box. Which widget to use?

    For example:

    If I want two empty answer boxes and the third empty box for instructor comments.

    Name two examples of fruit trees: Text Box 1 (For Answer 1), Text Box 2 (For answer 1), Text Box 3 (For instructor comments - student leave blank)

    Do yo mean something like this:

    If yes, you can simple use the Check Box field and enable the "Allow other" option:

    You can consider also the configurable list widget:

    You can add it from the widgets tab:
