If I understand you correctly, you are not getting the email notifications from JotForm right?

Your email is not on the bounce list. I checked the mail logs and I can see that the submissions were sent properly. Here are the mail logs:

[2015-05-05 17:57:48] Notification Test SENT to sales@e********orent.com from noreply@jotform.com via AMAZONSES

[2015-05-05 17:56:34] Notification Test SENT to sales@e********orent.com from noreply@jotform.com via AMAZONSES

    [2015-05-05 16:12:06] Submission #306665926061456211 Form #51246413076147 SENT to sales@e********orent.com from noreply@formresponse.com via AMAZONSES

I checked your notifier settings and I can that it is configured correctly. Please check your spam folders. I would advised you to please check if there are any spam filters on your email program or email client. You can also try white-listing JotMail IP addresses.

By the way, please go to the direct form and fill up the form so you can test it properly.

It worked 1 of 5 times trying it. I am not sure why it worked the first time but not any other time. If I change the email address to a GMail account, it works fine. Its odd. I don't have anything in spam or junk folders at my .Com email.

In that case, did you contact your email provider to request them to add JotMail's IP Addresses to their whitelist?

Another good solution would be to use Mandrill SMTP to send emails from you own email address, please check this guide: http://www.jotform.com/help/236-How-to-use-Mandrill-to-Send-Emails-From-Your-Own-Email-Address
