Form from form central to JotForm

Hello, 

I recently merged my forms from form central to jotform and I'm loving how this works. We'll be upgrading our account if I can get this to work right! I can not for the life of me get "Event Reservation Request" to work in jotform. When I click Preview it doesn't do anything. It works just fine in form central. Any ideas?

You can restore the form form the trash in your account:
