Why the letters or characters I type on my form do not show up?

If you type on the filling fieds of my form the typed letters do not show up. When you type from computer it does not show up what you type, it only shows up what you type from a smart phone. Can you please fix it? I have tried all the browsers, and on different computers but the problem is the same.

Please go to the link I provide below, click on the CATALOGO tab and then drag it down until you see the form. Then try to type something on.
