I was able to trace and existing JotForm account with username tonybfaire
associated to the email address you used on this post.
If in case you had forgotten the login password, you can reset the password using the
"Forgot Password" link in the login page https://www.jotform.com/login
the reset instruction will be sent via the email to the email address you used on this post.
