Can we create a form, embed it into an email that will be mail merged and send to a mailing list?

    We communicate regularly with our clients via Mail Merging info into an email. We would like to create a form with a couple of radio button options to gather info back from the client. Is there a way to accomplish this task in Outlook?

    Unfortunately, embedding a form into an email message is not possible. Some programs allow you to do that, but you have to consider the program that is used by the receiving party or your client to read the email. Gmail and other free email services will omit the codes and there's no way someone can submit the form from there.

    I would recommend adding the form link instead and let your clients open and fill it. If you want to prepopulate the form based on the information in your current mailing list, you can do a mail merge.

    For example: 

    http://www.jotformpro.com/form/51264441352953?email3=<<email>>

    Where email3 is the field name of your email field.

    Where <<email>> is the merge tag for your email field in outlook or any program.
