    I am interested in possibly doing more forms with you.  I had this form working before we upgraded our site.  Now with the upgrade I receive an error There are errors on the form. Please fix them before continuing.

    I removed the captula and that was not the problem. 

     

    Can you please help?  I also tried using IFrame and have the same problem.

    I suggest you re-embed first your jotform http://www.jotform.us/form/50226735317149 to your website using its iframe embed code

    Make sure you removed first any previous embedded script code of the form on the website before embedding again using the iframe embed.
