How can I get the Upload a File button to work?

    When I add it to my form and then test it on the website, I get an error saying the file failed.  I made sure the file is within the size and the file extension is one that is listed (jpg).  I also tried a Word document and it would not work. Suggestions?

     

    Form is on the school's website.  Under the Middle School Links/Program Reviews/Program Review Evidence Submission Form.

    I checked your form and made a submission to understand the problem. I successfully uploaded images (jpg.). Can you please re-check it and let us know?

    Just checked.  It is working when I am using Mozilla but not Internet Explorer 11 (I will check for upgrades and check settings).  THANK YOU!
