Unable to preview form?

    Hello, 

     

    I transfered a form Entitled "Event Reservation Request" from forms central to jot form. When I go to preview, it does nothing. The "Save" button goes around and around forever and does nothing. I can not test this form or obtain the URL. Any ideas? It worked just fine in FormsCentral. Thank you for your help!
