I set-up Jotform for 3 Facebook pages a couple of days ago. As soon as I set them them up there were no problems.
This morning however I tried testing the forms. Sometimes it works fine, other times it shows a 404 error. The notification/auto-reponse emails are also delayed.
Appreciate your help to make sure the form is reliable.

I have checked your Facebook page and I am getting warning "This content is currently unavailable".
Possible reason for this is that you have set your page to private and it should be public.
However if you are still getting 404 error I would suggest to try to re-embed your form in facebook and see if that helps you. You may also like to check the following guide which should help you embed your form in Facebook page: http://www.jotform.com/help/22-Adding-Form-to-Facebook-Pages
