I keep getting my form has been disabled

    whenever i create a new form and i input an email and password i keep getting the reply form has been disabled without getting the email and pass to my box. pls what can i do???

    Creating a login form (with username and password) is a direct violation to the JotForm Terms of Use.

    Your form was being disabled all the time due to this reason.

    Do not create such form anymore to avoid getting suspended.
