One of the people that's trying to fill out my form is running to this issue. I've been told that he's used different browsers and computers but the same problem still is reoccuring. Same thing is happening on the "Post Event Report" form where they are failing to be able to upload pictures.

I checked your form and tried uploading files. However, I am not able to replicate your issue because the files are successfully uploaded.

Thanks for getting back to me. I've tried myself as well and I can't seem to see what's the issue. I don't know if location has anything to do with it but I'm out in Oregon and the guy that's having a problem is out in Florida. I've been notified that he's been trying to fill out this form using 4 different computers and the same problem comes up.

I have also tried replicating the problem on various browser but my files upload successfully,
could you kindly ask the user which browser they are using that resulted in the failed attempts, and which OS it is in the failed scenarios
